Product Information
Encoraf (Encorafenib) 75mg is an oral BRAF kinase inhibitor used in the targeted treatment of BRAF V600E or V600K mutation-positive unresectable or metastatic melanoma, BRAF V600E mutation-positive metastatic colorectal cancer, and BRAF V600E mutation-positive metastatic non-small cell lung cancer. It works by blocking the constitutively activated BRAF V600 kinase, shutting down the overactive MAPK signalling pathway that drives uncontrolled tumour cell growth and survival. Encoraf is always given in combination with a partner agent such as binimetinib or cetuximab. Manufactured by Everest Pharmaceuticals Ltd., Bangladesh and globally supplied by Mediaid Pharmacy with worldwide shipping.

How Encoraf (Encorafenib) Works
Encoraf contains Encorafenib, a potent and selective inhibitor of the BRAF kinase. BRAF is a key signalling protein within the RAS-RAF-MEK-ERK (MAPK) pathway, which normally transmits controlled growth signals from the cell surface to the nucleus. When the BRAF gene carries a V600E or V600K mutation, the resulting kinase becomes permanently switched on, driving continuous proliferation signals that allow tumour cells to grow and survive without normal regulation.
Encorafenib binds the mutated BRAF kinase with very high potency and an unusually long target residence time, meaning it stays bound to the enzyme far longer than earlier-generation BRAF inhibitors. This sustained inhibition suppresses downstream MEK and ERK phosphorylation, halting the growth signal at its source and triggering tumour cell death in BRAF V600-mutant cancers.
Because tumours can escape single-agent BRAF blockade by reactivating the pathway further downstream, Encoraf is given in combination: with the MEK inhibitor binimetinib in melanoma and non-small cell lung cancer, and with the anti-EGFR antibody cetuximab in colorectal cancer, where EGFR feedback signalling is the dominant escape route. This dual blockade produces deeper, more durable pathway suppression and improves tolerability compared with BRAF inhibition alone.
Indications for Encoraf (Encorafenib)
Encoraf (Encorafenib) is indicated for the following conditions, in each case only where a BRAF V600 mutation has been confirmed by a validated diagnostic test:
|
Unresectable or Metastatic Melanoma In combination with binimetinib, for adult patients with unresectable or metastatic melanoma carrying a BRAF V600E or V600K mutation. This combination was established in the pivotal COLUMBUS trial and is a standard-of-care targeted regimen for BRAF-mutant advanced melanoma. |
Metastatic Colorectal Cancer (Previously Treated) In combination with cetuximab, for adult patients with BRAF V600E mutation-positive metastatic colorectal cancer after prior systemic therapy. BRAF V600E colorectal cancer carries a historically poor prognosis, and this chemotherapy-free doublet was the first targeted regimen approved for it. |
|
First-Line Metastatic Colorectal Cancer In combination with cetuximab and mFOLFOX6 chemotherapy for previously untreated BRAF V600E mutation-positive metastatic colorectal cancer, based on the BREAKWATER trial. This regimen extends targeted therapy into the first-line setting for this high-risk molecular subgroup. |
Metastatic Non-Small Cell Lung Cancer In combination with binimetinib, for adult patients with metastatic non-small cell lung cancer carrying a BRAF V600E mutation. This provides an oral targeted option for a small but clinically distinct subset of lung adenocarcinoma patients. |
Benefits of Encoraf (Encorafenib) 75mg
|
Precision Targeted Therapy Encoraf acts on a specific, testable genetic driver rather than dividing cells indiscriminately. Patients selected by BRAF V600 testing receive a therapy matched to the biology of their tumour, which is the defining advantage of precision oncology over conventional chemotherapy. |
Prolonged Target Binding Encorafenib remains bound to the mutant BRAF kinase substantially longer than earlier BRAF inhibitors. This extended residence time supports sustained pathway suppression across the full once-daily dosing interval. |
Three Approved Tumour Types Encorafenib is approved across BRAF V600-mutant melanoma, colorectal cancer, and non-small cell lung cancer, making it one of the most broadly applicable BRAF inhibitors available to oncologists managing BRAF-driven disease. |
|
Convenient Once-Daily Oral Capsule Encoraf is taken once daily by mouth, with or without food, and can be managed at home. There is no infusion requirement for the Encorafenib component, reducing hospital visits and treatment burden for patients on long-term therapy. |
Affordable Alternative to Braftovi Encoraf delivers the same active molecule as Braftovi by Array BioPharma and Pfizer at a fraction of the originator price, making sustained BRAF-targeted therapy realistically affordable for self-funding patients and health systems in emerging markets. |
GMP-Certified Oncology Manufacturing Produced by Everest Pharmaceuticals Ltd. in a dedicated oncology manufacturing facility operating to current Good Manufacturing Practice standards, with batch-level quality control ensuring consistent potency and purity in every pot. |
How to Take Encoraf (Encorafenib)
Route: Oral. Swallow the capsules whole with water. Do not open, chew, or crush the capsules.
Melanoma dose: 450mg once daily, which is six 75mg capsules, taken together with binimetinib 45mg twice daily, continued until disease progression or unacceptable toxicity.
Non-small cell lung cancer dose: 450mg once daily, which is six 75mg capsules, taken together with binimetinib 45mg twice daily.
Colorectal cancer dose: 300mg once daily, which is four 75mg capsules, taken together with cetuximab, with or without mFOLFOX6 chemotherapy depending on the treatment line.
With food: Can be taken with or without food. A high-fat meal lowers peak concentration but does not meaningfully change total exposure. Taking the dose at the same time each day is recommended.
Missed dose: Take the missed dose as soon as it is remembered, unless the next scheduled dose is 12 hours away or less, in which case skip it. Do not double the dose. If vomiting occurs after a dose, do not take an extra dose; resume at the next scheduled time.
Renal impairment: No dose adjustment is required for mild to moderate renal impairment. Data are limited in severe renal impairment.
Hepatic impairment: No dose adjustment is required in mild hepatic impairment (Child-Pugh Class A). A recommended dose has not been established for moderate (Child-Pugh B) or severe (Child-Pugh C) hepatic impairment.
Paediatric use: The safety and effectiveness of Encoraf in paediatric patients have not been established.
Pack Supply Calculation for Ordering
Each pot contains 90 capsules of 75mg. Because Encoraf is dosed at multiple capsules per day, one pot does not cover a full month:
At 450mg daily (melanoma, NSCLC): 6 capsules per day. One pot lasts 15 days. A 30-day month requires 2 pots (180 capsules).
At 300mg daily (colorectal cancer): 4 capsules per day. One pot lasts 22 to 23 days. A 30-day month requires 2 pots, with a partial carry-over into the following month.
Please account for this when planning continuity of supply, particularly where shipping transit times are long. Contact Mediaid Pharmacy to arrange multi-pot or multi-month orders.
Monitoring and Dosage Adjustments
Structured monitoring is essential throughout Encoraf therapy. Patients require regular dermatological examination before treatment, every two months during treatment, and for up to six months after the final dose, because BRAF inhibitors are associated with new primary cutaneous malignancies including cutaneous squamous cell carcinoma, keratoacanthoma, and new primary melanoma. Left ventricular ejection fraction should be assessed at baseline, after one month, and every two to three months thereafter when Encoraf is combined with binimetinib.
Additional monitoring includes liver function tests (ALT, AST, bilirubin) at baseline and monthly, serum creatine phosphokinase and creatinine for rhabdomyolysis, ECG and serum electrolytes including potassium and magnesium for QT interval prolongation, and ophthalmological assessment at any report of visual disturbance, given the risk of uveitis and, in combination with binimetinib, serous retinopathy. Patients should also be assessed for signs of venous thromboembolism, bleeding, and interstitial lung disease. Dose reduction, interruption, or discontinuation is managed by the oncologist according to the graded severity of any adverse reaction.
Drug Interactions and Important Information
Encorafenib is primarily metabolised by CYP3A4. Concomitant use of strong or moderate CYP3A4 inhibitors such as ketoconazole, itraconazole, clarithromycin, ritonavir, and grapefruit juice should be avoided, as they substantially increase Encorafenib exposure and the risk of toxicity. If co-administration is unavoidable, dose reduction under oncologist supervision is required. Conversely, strong or moderate CYP3A4 inducers such as rifampicin, carbamazepine, phenytoin, and St John’s Wort should be avoided because they reduce Encorafenib exposure and may compromise efficacy.
Encorafenib is itself an inducer of CYP3A4, CYP2B6, and CYP2C9. Clinically, this means it can reduce the effectiveness of hormonal contraceptives, so women of childbearing potential must use an effective non-hormonal contraceptive method during treatment and for at least two weeks after the final dose. Caution is also required with warfarin and other narrow therapeutic index CYP2C9 substrates.
Because Encorafenib can prolong the QT interval, concurrent use with other QT-prolonging medicines such as certain antiarrhythmics, macrolide antibiotics, fluoroquinolones, and antipsychotics requires ECG monitoring and correction of electrolyte abnormalities. Always give your oncologist a complete list of all prescription medicines, over-the-counter products, supplements, and herbal remedies before starting Encoraf.

Who Should Use Encoraf (Encorafenib)?
Encoraf is intended for adult patients with a molecularly confirmed BRAF V600E or V600K mutation in unresectable or metastatic melanoma, or a confirmed BRAF V600E mutation in metastatic colorectal cancer or metastatic non-small cell lung cancer. It is prescribed exclusively by medical oncologists working within a service capable of molecular tumour profiling, cardiac imaging, and ophthalmological and dermatological review.
Encoraf is not appropriate for patients whose tumours are BRAF wild-type, for patients with hypersensitivity to Encorafenib, or for patients who are pregnant or breastfeeding. It requires caution in patients with pre-existing cardiac dysfunction, significant hepatic impairment, uncontrolled electrolyte abnormalities, or a history of thromboembolic disease. Because the partner agent is integral to the regimen, patients must also be able to access and tolerate binimetinib or cetuximab alongside Encoraf.

Frequently Asked Questions About Encoraf (Encorafenib)
What is Encoraf (Encorafenib) 75mg used for?
Encoraf (Encorafenib) is a targeted BRAF kinase inhibitor used to treat cancers driven by a BRAF V600 mutation. It is indicated for unresectable or metastatic melanoma with a BRAF V600E or V600K mutation in combination with binimetinib, for BRAF V600E metastatic colorectal cancer in combination with cetuximab (with or without mFOLFOX6 chemotherapy), and for BRAF V600E metastatic non-small cell lung cancer in combination with binimetinib. A BRAF mutation test is required before treatment can begin.
How many Encoraf capsules are taken per day?
It depends on the cancer being treated. For melanoma and non-small cell lung cancer the dose is 450mg once daily, which is six 75mg capsules taken together. For colorectal cancer the dose is 300mg once daily, which is four 75mg capsules. All capsules for the day are taken as a single once-daily dose, with or without food, at approximately the same time each day. Your oncologist will confirm the correct dose for your diagnosis.
How long does one 90-capsule pot of Encoraf last?
At the 450mg melanoma and lung cancer dose of six capsules daily, one pot of 90 capsules lasts 15 days, so two pots are needed per 30-day month. At the 300mg colorectal cancer dose of four capsules daily, one pot lasts 22 to 23 days. Please factor this into your order quantity and shipping schedule so treatment is not interrupted. Mediaid Pharmacy can arrange multi-pot and multi-month supply.
Can Encoraf be taken on its own without another medicine?
No. Encoraf is a combination therapy and should not be used as a single agent at the 450mg dose, which is not well tolerated without a partner drug. It is given with binimetinib for melanoma and lung cancer, or with cetuximab for colorectal cancer. If binimetinib has to be paused, the Encorafenib dose is reduced to 300mg once daily for that period under oncologist direction and then returned to 450mg when binimetinib resumes.
Do I need a BRAF mutation test before taking Encoraf?
Yes, this is essential. Encoraf only works in tumours carrying a BRAF V600 mutation, and it is not indicated for BRAF wild-type disease. In tumours without the mutation, BRAF inhibitors can paradoxically activate the growth pathway and may accelerate tumour progression. Your oncologist will confirm BRAF V600 status from a tumour tissue sample or a plasma test before prescribing.
Is Encoraf the same as Braftovi (Encorafenib by Pfizer)?
Encoraf contains the same active pharmaceutical ingredient, Encorafenib, in the same 75mg capsule strength as Braftovi by Array BioPharma and Pfizer. It is manufactured by Everest Pharmaceuticals Ltd. in Bangladesh and offered at a substantially lower cost, giving patients access to the same targeted mechanism of action without originator pricing.
What are the main side effects and safety concerns with Encoraf?
Common side effects include fatigue, nausea, vomiting, diarrhoea, joint pain, skin thickening, rash, hand-foot syndrome, headache, and anaemia. Serious concerns requiring active monitoring include new skin cancers, reduced heart function when combined with binimetinib, liver enzyme elevation, QT interval prolongation, blood clots, bleeding, and eye inflammation. Regular skin checks, cardiac scans, liver tests, and eye reviews are part of standard care on this treatment.
